On a construction site the workforce turns over constantly and most of it does not work for you. A scaffold tag that is out of date, an excavation without a barrier, a load rigged by a crew who arrived on Monday: these are seen every day by someone who has thirty seconds to do something about it. What decides whether the site gets safer is not whether the observation happens. It is whether anything downstream of it does.

Observations stop at the card

Unsafe conditions are written on a card handed in at the end of shift, by which time the crew that created the condition has moved to another work front.

Without a per-contractor count there is no way to tell a contractor genuinely reporting from one that has stopped, and both look identical in the monthly total.

Corrective actions have no owner

A condition that needed a barrier installed or a permit reissued is logged as an observation and never becomes work assigned to anyone.
